Physical Therapist - Brooklyn, NY We are hiring an experienced Physical Therapist for a busy multi-specialty outpatient health center. This is a true full-time W2 role with above-market pay.
Responsibilities
- Perform initial evaluations and build individualized treatment plans
- Treat a diverse caseload from pediatrics to geriatrics, including orthopedic and post-surgical rehab
- Supervise and coordinate care with a PTA
- Collaborate with primary care and specialty providers in a multidisciplinary team
- Track progress, adjust plans of care and educate patients and families
- Document all visits accurately in the EMR
Qualifications
- Active New York State PT license
- Active Medicare and Medicaid provider IDs (required)
- Outpatient, acute care, home health or hospital PT experience preferred
- Comfortable with EMR documentation
- Available for the Sunday and evening schedule above
- Strong patient-centered communication skills
