Job Description
A client of Insight Global in the healthcare technology sector is seeking a Program & Care Coordinator to help grow and scale a grant funded health initiative across the state of Tennessee. This program is designed to engage at risk population for heart disease and enroll them into this program to give them access to a wearable ECG device to catch heart disease early and save lives. The Program & Care Coordinator is the second full-time seat on TN Heart Access and reports to the Program Director. Where the Program Director owns the award, the State relationship, and the participating practices, this role owns the patient journey: making sure every person who enrolls moves through screening, results, cardiology input, and follow-up at the right time, and that every step is captured accurately in the program's data. It's a role for a clinician with a real heart for patient advocacy who also takes genuine satisfaction in a clean, complete dataset. If you've worked as a clinical nurse coordinator or a clinical trial / research nurse coordinator, this will feel familiar.
CARE COORDINATION & PATIENT ADVOCACY
- Serve as the consistent, go-to contact for every enrolled patient, from enrollment through five years of follow-up
- Walk patients through each step of the program in plain language: what the wearable ECG is, how to wear and return it, how results come back, and what happens next
- Make sure patients get to the right appointments at the right time: support their scheduling of follow-up visits, cardiology consults, and re-monitoring, and close the loop on anything missed
- Drive patient adherence (full device wear, timely return, completed follow-up) and find and remove the barriers that get in the way, from transportation to cost to confusion about next steps
- Ensure urgent findings reach the treating PCP immediately, with the escalation documented
- Keep patients on their defined re-monitoring cadence and support retention across the five-year program
- Work hand-in-hand with practice staff (front desk, nurses, and PCPs) on patient-level questions so each patient's care keeps moving
DATA, DOCUMENTATION & REGISTRY ABSTRACTION
- Abstract the required data from each practice's EHR at every stage of the program: eligibility and enrollment, screening and results, follow-up, and recurring check-ins on a set cadence
- Confirm eligibility at enrollment against program criteria (age, documented comorbidities, and absence of qualifying cardiac symptoms) using the medical record
- Know where things live in the chart (problem lists, encounter notes, medications, labs, imaging, outside records) and navigate several EHR platforms across participating practices
- Request and track medical documentation from outside providers (specialists, hospitals, other clinics) when what's needed isn't in the primary chart
- Enter registry data accurately and on time; run quality checks, resolve gaps and discrepancies, and keep a clean audit trail
- Partner with our data and analytics lead to keep definitions consistent and support outcome reporting to the Tennessee Department of Health
