Social Worker/Concierge– Full-Time Cedar Haven Healthcare Center | Lebanon, PA Cedar Haven Healthcare Center is seeking a full-time Social Worker/Concierge to join our compassionate team in Lebanon, PA. In this role, you will serve as a key advocate for residents - supporting their emotional well-being, guiding families, and helping ensure a smooth transition into and throughout their stay with us.
Key Responsibilities
- Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals
- Participate actively in pre-admission and admission processes
- Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need
- Document behavioral and psychosocial updates in accordance with facility and regulatory requirements
- Lead individual and group support as needed
- Develop and grow volunteer programs tailored to resident needs
- Orient new residents and families to the center to ensure a smooth transition
