SOCIAL WORKER
Make a difference every day! We are seeking a compassionate and motivated Social Worker to join our skilled nursing team. This position plays an important role in supporting our residents and families while promoting resident-centered care and quality of life.
RESPONSIBILITIES
- Complete social service assessments and documentation
- Participate in care planning and interdisciplinary meetings
- Assist residents and families with psychosocial needs
- Coordinate discharge planning and community resources
- Advocate for resident rights and preferences
- Maintain accurate and timely documentation

QUALIFICATIONS
- Degree in Social Work or related field as required by applicable regulations
- Minimum 1 year of Skilled Nursing/Long-Term Care experience preferred
- Strong communication and organizational skills
- Compassionate, professional, and team-oriented
- PointClickCare (PCC) experience preferred
