Responsibilities
- Complete and submit accurate MDS assessments
- Coordinate and maintain individualized resident care plans
- Collaborate with the interdisciplinary team to support resident outcomes
- Ensure compliance with CMS, Medicare, Medicaid, and state regulations
- Assist with quality improvement initiatives and survey readiness
- Provide education and support related to MDS and care planning processes
Qualifications
Qualifications
- Current Iowa RN or LPN license
- Minimum 2 years of long-term care experience
- Strong assessment, documentation, and communication skills
- MDS experience preferred
- PointClickCare experience preferred
