Position Summary We are looking for an SLP who is passionate about providing high-quality, resident-centered care and helping patients improve their communication, cognition, swallowing, and overall quality of life.
Responsibilities
- Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders
- Deliver evidence-based therapy services in accordance with physician orders and facility policies
- Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive patient care
- Educate patients, families, and caregivers on treatment goals, home programs, and progress
- Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines
- Participate in care plan meetings and contribute to discharge planning as needed
Qualifications
- Master’s Degree in Speech-Language Pathology from an accredited program
- Current State Licensure or eligibility for licensure as a Speech-Language Pathologist
- Current Certificate of Clinical Competence (CCC-SLP) from ASHA
- CFY candidates welcome to apply
- Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required
- Strong interpersonal, organizational, and communication skills
- Compassionate, dependable, and patient-focused approach to care
