Responsible for overseeing and coordinating services for individuals of all ages with Autism and Developmental Disabilities. This can be a full-time or part-time position. Hours will be split between Livingston and Grundy counties. Must have a valid driver's license and ability to pass a background check. Federal mileage reimbursement for travel. - Meet with individuals and their family at their residence or within the community to provide service coordination (case management)
- Develop and implement individual service plans for clients with developmental disabilities
- Coordinate with behavioral health professionals to ensure comprehensive care
- Build and maintain positive relationships with clients, families and community partners
