**This is NOT an IT position**
**This is an In Office/On Site position, Monday through Friday and requires on call rotation**
This role operates administratively in a variety of different Service Departments. Rather than institutionalizing individuals who have these physical and mental health needs, we provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.
Service Programmers provide a fresh look at the services we provide. Within the team, you’ll join the mission of empowering people to reach individual achievement across the spectrum of life. If you’re passionate about empowering people to reach their goals and help them strive for more, we want you to join our team. As a core team member, you’ll be responsible for monitoring services, creating new strategies to reach goals, and coordinate services in a way that empowers people to meet and exceed their goals. Your consistent leadership will provide your team with the trusted foundation they need to make a difference in the lives of the people we serve.
What Winning Looks Like:
While it’s not a competition, we do recognize that each person wants to ‘win’ at life; and you’re a central part of someone else’s wins – as well as your own! In this role, you’ll be responsible to:
- Team Leadership & Culture: Build and maintain a positive team culture through active staff engagement, regular team meetings, coaching, and ongoing support. Participate in on-call responsibilities and provide short-notice coverage when needed to maintain continuity of services.
- Resource & Operational Management: Oversee key resources and operational needs, including vehicle scheduling, individual budgets, representative payee coordination, medication check-ins, appointment scheduling, transportation, and other supports necessary for effective service delivery.
- Person-Centered Service Coordination: Coordinate with individuals, families/guardians, case managers, Medicaid Community Based Care Managers (CBCM), and the Interdisciplinary Team (IDT) to develop individualized plans, establish goals and outcomes, maintain authorizations and required assessments, and monitor individual progress.
- Compliance & Documentation: Ensure services comply with applicable agency policies, laws, regulations, and industry standards. Maintain complete, accurate, and timely documentation in the approved EMR, including service records, incident reports, investigations, individual interactions, goal progress, and changes to services or rights.
- Advocacy & Community Partnerships: Advocate for individuals to have meaningful choice, independence, dignity, and control over their lives while ensuring services reflect their needs, preferences, and goals. Collaborate with internal teams and community partners to strengthen resources, relationships, and opportunities available to individuals served.
