Full-time
Weekend Schedule with Flexible Weekdays
Work four days, choose your weekdays. This role covers Saturday and Sunday, and you pick the two weekdays that fit your life. We prefer 10-hour shifts, which means a four-day week with three days off, but we’re also open to 8-hour shifts. If you’ve been looking for a schedule that leaves room for family, school, or simply more time to recharge, this could be the one.
About the Role
As a Withdrawal Management Assessor, you’ll meet patients at one of the most important moments in their recovery: the beginning. You’ll complete substance use disorder screenings and comprehensive assessments, help shape each patient’s stabilization and discharge plan, and guide them toward the next right step, whether that’s MnTC’s 245G program or another appropriate level of care.
You’ll work as part of an interdisciplinary team alongside nursing staff, medical providers, counselors, and program leadership, and you’ll partner with referral sources and community organizations to make sure every transition is safe, timely, and person-centered.
What You’ll Do
- Screen patients at admission and complete comprehensive substance use disorder assessments and summaries before a medically stable discharge and within 72 hours of admission (Minn. Stat. 245F.06).
- Gather patient history and collateral information with appropriate consent, and make clinical, referral, and level-of-care recommendations within your credentials and scope of practice.
- Engage patients with trauma-informed, culturally responsive, person-centered care, helping them understand program expectations, available services, and next steps.
- Plan and lead recovery-focused groups, educational sessions, and structured activities.
- Coordinate referrals and transitions to the next level of care, and keep in contact with county workers, probation and parole officers, treatment providers, and other authorized partners.
- Collaborate with the care team through clinical consultations, interdisciplinary discussions, and staff meetings.
- Maintain accurate, timely documentation, including assessments, progress notes, group records, individualized plans, referrals, authorizations, and DAANES admission and discharge reporting.
- Help create a welcoming, organized, therapeutic environment that supports patient safety, stabilization, and recovery.
- Protect patient confidentiality and report safety concerns, suspected abuse or neglect, incidents, and regulatory concerns within required timelines.
- Admit or discharge patients when designated under MnTC’s 245F policies. (Medical stability and transfer decisions remain with nursing and medical staff.)
- Take part in training, supervision, quality assurance, audits, and incident reviews, and seek consultation when a situation goes beyond your role or competence.
- Follow Minnesota Statutes Chapter 245F, Chapters 245G and 245I when applicable, DHS requirements, professional licensing standards, payer requirements, and MnTC policies.
