About the Role of LPN for SNF:
As an LPN with Deer Meadows, you will play a vital role in our Skilled Nursing Facility (SNF) team, providing high-quality patient care to our residents.
LPN for SNF Responsibilities:
- Providing direct patient care in accordance with our facility's policies and procedures
- Conducting vital signs, wound care, and other basic nursing procedures
- Maintaining accurate and timely records of patient care and treatments
- Collaborating with the interdisciplinary care team to develop and implement individualized care plans
- Providing emotional support and comfort to our residents and their families

LPN for SNF Requirements:
- Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
- Prior experience in a long-term care setting preferred
