Job Summary:
Brookside Care Center, a leading healthcare provider in Stockton, CA, is seeking a motivated and compassionate Physical Therapy Assistant to join our interdisciplinary team. As a Physical Therapy Assistant, you will play a vital role in delivering high-quality patient care and contributing to the recovery and well-being of our residents.
Responsibilities:
- Assist physical therapists in planning, implementing, and evaluating treatment plans for patients with a range of medical conditions and injuries
- Conduct physical therapy treatments, including exercises, stretches, and modalities, under the supervision of a licensed physical therapist
- Monitor patient progress and report changes to the physical therapy team
- Communicate effectively with patients, families, and healthcare staff to ensure seamless care transitions
- Maintain accurate and thorough records of patient treatments and progress
- Participate in patient education and family training programs to promote independence and successful discharge
Requirements:
- Graduate of an accredited Physical Therapy Assistant program
- Possess a current Physical Therapy Assistant license to practice in the state of California

- Ability to lift, transfer, and position patients with varying abilities and needs
