Responsibilities:
- Provide pharmaceutical care to patients, including medication therapy management, patient education, and medication reconciliation
- Review and interpret medication orders, dispense medications, and maintain accurate medication records
- Participate in the development and implementation of pharmacy programs and services to improve patient outcomes
- Collaborate with healthcare teams to ensure seamless patient care
- Maintain compliance with regulatory agencies and hospital policies
Requirements:
- New York State Pharmacist License (current and unrestricted)
- Bachelor's degree in Pharmacy or related field
- Minimum 1 year of experience as a practicing pharmacist
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong attention to detail and accuracy
