This position requires a Monday through Friday schedule with Day Shift hours from 08:00 to 16:30. No weekends or holidays are required.
Job Description
The Case Manager, RN is responsible for overseeing all medical and surgical patients on each unit, including ICU and IMC settings. The role involves care coordination, discharge planning, and ensuring compliance with Medicare and Medicaid services. Additionally, the nurse will use the Epic charting system to document patient care and manage appeals and denials.
Responsibilities
- Coordinate care and discharge planning for patients.
- Conduct continued stay reviews and determine medical necessity per evidence-based guidelines.
- Manage appeals and denials as necessary.
- Document care and patient management activities in the Epic system.
- Collaborate with an interdisciplinary team, which includes social services and interpretation services.
Required Experience
- Minimum of 2 years of charge experience preferred.
- Experience in acute hospital settings is required.
- Must be able to manage a patient ratio of up to 25 patients.
Certifications/Licensure
- Active RN license in the state of Maryland or compact license.
- Basic Life Support (BLS) certification required.
- Care Management certification preferred.
- CPI certification must be obtained before starting.
