Job Description
As a Registered Nurse specializing in Case Management, you will oversee patient care coordination, discharge planning, and ensure adherence to care protocols. This role requires acute care experience, with an emphasis on managing patient admissions and discharges, care coordination, and understanding benefits eligibility.
Responsibilities
- Manage patient case loads and facilitate smooth transitions of care.
- Coordinate with interdisciplinary teams to optimize patient outcomes.
- Develop discharge plans in accordance with CMS guidelines.
- Perform documentation using the Epic charting system.
- Assist with prior authorizations and appeals as needed.
Required Experience
- Minimum of 2 years of acute care experience.
- Must be current in clinical skills within the past year.
- Experience with HMOs, IPAs, and managing advance directives.
Certifications/Licensure
- Current and valid Registered Nurse license.
- Compact license must be in hand at submission.
