Job Description
We are seeking a dedicated Licensed Practical Nurse to provide high-quality care in a Long-Term Care facility. You will be part of a team that serves a diverse resident population, including skilled nursing, rehabilitation, and dementia care. The facility houses 300 total beds divided into six units, with a focus on person-centered care.
Responsibilities
- Administer medications and treatments as prescribed.
- Monitor patient vital signs and perform assessments.
- Assist with activities of daily living and provide personal care.
- Conduct lab draws and specimen collections.
- Provide wound care, including measurement and documentation.
- Collaborate with interdisciplinary team members for optimal patient outcomes.
Required Experience
Prior experience in Long-Term Care is preferred, along with knowledge of using the PCC electronic medical record system.
Certifications/Licensure
BLS certification is required. A valid LPN license is necessary for this position.
