Job Description
The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care to residents on the assigned unit while following established policies and procedures under the supervision of a Registered Nurse (RN). The LPN will demonstrate the knowledge and skills necessary to deliver effective care tailored to the age-specific needs of adult and geriatric populations.
Responsibilities
- Collect and evaluate data regarding residents' physical and psychosocial conditions, and communicate findings to the RN and interdisciplinary team.
- Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and interventions needed for optimal resident outcomes.
- Collaborate with residents, families, and the healthcare team to ensure comprehensive care and customer satisfaction.
- Maintain up-to-date knowledge of nursing practice and assist in mentoring and orienting new staff members.
Required Experience
1 year of experience as a Licensed Practical Nurse in a long-term care facility.
Certifications/Licensure
- Current NY LPN Licensure
- BLS certification (through AHA or RQI)
