What You'll Do
As a PRN Physical Therapist, you'll help residents improve strength, balance, mobility, and functional independence through individualized, evidence-based rehabilitation.
Your responsibilities will include:
- Perform comprehensive physical therapy evaluations and assessments.
- Develop individualized treatment plans based on each resident's clinical needs and functional goals.
- Provide skilled physical therapy interventions to improve mobility, balance, strength, transfers, and safety.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Collaborate with Physical Therapist Assistants, nursing staff, and interdisciplinary team members to coordinate care.
- Educate residents, families, and caregivers on mobility techniques, fall prevention, and safe functional activities.
- Maintain accurate and timely clinical documentation in accordance with professional standards and applicable regulations.
- Promote a positive, supportive, and patient-centered rehabilitation experience.
What We're Looking For
- Graduate of an accredited Physical Therapy program.
- Current, active Pennsylvania Physical Therapist license or eligibility for licensure.
- Strong clinical assessment, treatment planning, and communication skills.
- Ability to work independently while contributing to a collaborative rehabilitation team.
- Compassionate, dependable, and committed to providing excellent patient care.
- Skilled nursing facility or geriatric rehabilitation experience preferred.
