Job Summary:

Responsibilities:

• Develop and implement comprehensive care plans for each resident's social services needs
• Hold meetings with patients/residents, families, and significant others as requested, to provide support and information to improve quality of life
• Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers
• Establish relationships and maintain contact and referral flow with community-based agencies and services

Qualifications:

• Bachelor's degree from an accredited school of Social Work or related field is required
• At least one year of supervised social work experience in a healthcare setting, working directly with individuals, is preferred
