Original job description
Description WESTSIDE IS LOOKING FOR A ELEVATE APP PROGRAM MANAGER! JOIN THE TEAM THAT MAKES A DIFFERENCE IN THE LOCAL COMMUNITY!
Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care. The Program Manager, Elevate APP Program, supports Westside Family Healthcare's community health initiatives, with a primary focus on implementing grant-funded Food Is Medicine (FIM) interventions that integrate nutrition and food access into patient care. This position is grant-funded. Continuation is contingent on the availability of grant funding. Working under the direction of the Chief Strategy Officer and alongside clinical teams, the Nutrition Services team, and community partners, the Project Manager helps design implementation workflows, solve problems as programs take shape, and ensure activities meet funder requirements. Current work centers on WFH's Food Is Medicine portfolio, including the HRSA Maternal Produce Prescription Program, for which WFH is the prime recipient, and the Delaware Rural Health Transformation Program Food Is Medicine project led by the University of Delaware. These projects operate within a multi-partner collaborative in which the University of Delaware coordinates research and evaluation and partner organizations have counterpart project managers. As funding and priorities evolve, the role may support additional Food Is Medicine and community health projects aligned with WFH's strategy. This role helps shape program direction and partner alignment by bringing implementation insight, data, and recommendations to WFH leadership. Final decisions on program direction, partner agreements, and strategy rest with leadership; the Project Manager helps carry them out efficiently and in compliance with each award.
