Original job description
About the Position Are you organized, motivated, and someone who thrives in a fast-paced environment? We are looking for a dependable Intake Coordinator to join our growing Home Health & Hospice team! This is a high-volume position that requires someone who can manage multiple responsibilities at once, prioritize competing tasks, and keep things moving throughout the day. Our intake department works with a continuous flow of incoming referrals, making organization, communication, and the ability to adapt quickly essential to success. As an Intake Coordinator, you will help coordinate new patient referrals, communicate with referral sources, and work closely with our clinical and administrative teams to ensure patients receive timely care. Healthcare experience is helpful, but we are open to candidates with strong administrative skills, a great work ethic, and a willingness to learn. What You'll Do
- Receive, review, and process incoming patient referrals.
- Manage multiple referrals and requests simultaneously while keeping track of pending items.
- Review referral information and follow up on missing or incomplete documentation.
- Assist with insurance eligibility and benefits verification.
- Communicate with physician offices, referral sources, patients, and families.
- Enter and maintain accurate patient information in the EMR system.
- Monitor pending referrals and proactively follow up on outstanding items.
- Adjust priorities throughout the day as new referrals and urgent requests come in.
- Help ensure a smooth and timely intake process.
What We're Looking For
- Previous administrative, customer service, or healthcare experience preferred.
- Healthcare intake, admissions, or home health experience is a plus, but not required.
- Bilingual in English and Spanish preferred.
- Comfortable using computers, email, and Microsoft Office.
- Strong communication and organizational skills.
- Excellent attention to detail and follow-through.
- Ability to multitask and manage several responsibilities at once.
- Comfortable working in a fast-paced, high-volume environment where priorities can change quickly.
- Ability to stay productive under pressure without losing accuracy.
- Positive attitude, initiative, and willingness to learn.
The Ideal Candidate
We are looking for someone who:
- Thrives in a busy work environment and enjoys staying productive.
- Can switch between tasks quickly without losing track of priorities.
- Knows how to determine what needs immediate attention and what can wait.
- Takes initiative and follows through without constant reminders.
- Stays calm, organized, and professional when multiple requests come in at once.
- Understands the importance of timely communication and getting things done.
This is not a slow-paced administrative position. The right candidate should enjoy staying busy, handling multiple responsibilities, and working in an environment where there is always something that needs attention. Why Join Our Team? Join a growing healthcare organization where your work helps connect patients with the care they need. You'll be part of a collaborative team with opportunities to learn, grow, and make a meaningful contribution every day.
