Position Summary

Upper Valley Haven is seeking a compassionate and strengths-based Service Coordinator to join our Community Service Coordination team, supporting individuals and families staying in local hotels as part of the Vermont Homelessness Response Continuum.

This grant-funded position is focused on providing consistent, individualized support through smaller caseloads of approximately 20 households per Service Coordinator and weekly client contact. The Service Coordinator builds trusting relationships with individuals and families experiencing homelessness, helps clients access resources and identify housing opportunities, and supports them in working toward permanent housing and their individual goals.

The position is based at the Haven’s Leahy Shelter and Resource Center when not working at local hotel sites.

Why join us?

In this role, you’ll have the opportunity to build meaningful relationships with individuals and families, work collaboratively with community partners, and help people navigate resources and opportunities that support their goals and movement toward permanent housing. You’ll also receive a funded, role-based learning and development plan, with clinical supervision available from an LICSW for those pursuing licensure.

  • Connect clients with appropriate resources and supports related to mental health, substance use, employment, primary care, and other needs.
  • Help clients access public benefits, including 3SquaresVT and Medicaid.
  • Support clients in identifying and working toward their individual goals while navigating complex service systems.
  • Use advocacy, de-escalation, crisis management, assessment, and problem-solving skills when responding to challenging situations.
  • Build collaborative relationships with community partners and partner agencies.

• Maintain thorough, timely, and professional documentation.

QUALIFICATIONS

Required:

  • Bachelor’s degree in social work, human services, psychology, or a related field.
  • Experience in social services, nonprofit organizations, homeless services, or a related setting.

  • Strong interpersonal, written, and verbal communication skills.
  • Strong assessment, ethical decision-making, problem-solving, and organizational skills.
  • Skills in advocacy, de-escalation, and crisis management.
  • Ability to work independently while collaborating effectively with Haven colleagues and community partners.
