Work collaboratively with supervisors, treatment teams, service providers, and community partners to address client needs.
Participate in treatment team meetings and contribute recommendations regarding client progress, service needs, and changes in status.
Coordinate referrals, facilitate communication among providers, and help clients navigate multiple services and systems.
Assist with developing or delivering educational materials, life-skills instruction, and group curriculum when needed.
Identify barriers to progress and work with clients and team members to develop practical solutions.

Maintain Accurate Documentation and Professional Standards

Complete timely, accurate, and thorough documentation of client contacts, assessments, service plans, interventions, and outcomes.
Ensure records meet agency, program, funding, accreditation, and credentialing requirements.
Follow applicable professional standards, agency policies, protocols, rules, and regulations.
Complete all required agency, accrediting body, and funder training.
