• Hours: Full-time position
• Travel: Ability to spend approximately 20% of the day driving to/from patient locations
• Schedule: Monday-Friday, 8am-5pm *no on-call or weekends

The Role Join our client's care team as a Home Health Physical Therapy Assistant, providing compassionate support to patients in their homes. Key Responsibilities

• Collaborate to provide management and delivery of patient care plans
• Support patients in Marion, IL and surrounding areas
• Meet critical deadlines for charting
• Self-manage time and schedule effectively

Qualifications

• Associate degree or higher
• 1+ years of experience in ANY of the following:
• Home Health Care
• Home Health
• 1+ years of experience in ANY of the following:
• Electronic Medical Record (EMR)
• Practice Partner EMR
• EHR Implementation
• Valid Regular Driver License (DL) certification/license in the state of Illinois (IL)
• Valid Physical Therapy Assistant certification/license in the state of Illinois (IL)
