We are looking for a Patient Care Coordinator to support referral intake, insurance documentation, and service setup for patients in Riverside, Missouri. This Long-term Contract position plays a key role in connecting patients, families, providers, and internal care teams to ensure services begin smoothly and remain properly authorized. The ideal candidate brings strong judgment, attention to detail, and the ability to manage time-sensitive coordination in a regulated healthcare environment.

Responsibilities:

  • Manage incoming referrals from physicians, hospitals, care facilities, and community partners, ensuring each case is processed accurately and promptly.
  • Collect and organize the records needed to confirm medical insurance eligibility, authorization status, and approval for care initiation.
  • Partner with clinical intake and interdisciplinary teams to coordinate admissions, required services, and readiness for new patient starts.
  • Review patient financial and coverage documentation to confirm benefits are clearly documented, including any out-of-pocket responsibility.
  • Maintain compliance with organizational procedures and applicable local, state, and federal requirements governing intake and admission activities.
  • Oversee scheduling and service coordination for new referrals, making sure ordered equipment, supplies, and related support are available when needed.
  • Track ongoing orders and authorizations for existing patients, including submitting renewal or updated requests to physicians as appropriate.
  • Respond to patient and family questions, investigate issues, and resolve coordination problems in a thorough and timely manner.
  • Prepare delivery-related paperwork and communicate clearly with clinicians, field staff, and support departments to keep patient services on schedule.
  • Experience coordinating patient referrals, admissions, scheduling, or related healthcare support activities.
  • Working knowledge of medical insurance verification, authorization processes, and patient coverage documentation.
  • Familiarity with patient medical records and the ability to handle confidential information with accuracy and discretion.
  • Ability to communicate effectively with patients, families, physicians, and cross-functional healthcare teams.
  • Strong organizational skills with the capacity to manage multiple cases, deadlines, and follow-up tasks at the same time.
  • Proficiency with healthcare software systems and standard office applications used for scheduling and documentation.
  • Background supporting durable medical equipment or similar service-based healthcare coordination is preferred.
