RN Care Coordinator

POSITION SUMMARY:

The RN Care Coordinators works collaboratively with the CCBHC team to coordinate services for consumers who are shared throughout programs. Care Coordinators ensure an inter-grated approach is taken for all consumers volunteering to participate in the CCBHC program. RN Care Coordinators will work with primary programs to ensure engagement in treatment programs to meet outlined goals. The RN Care Coordinators will provide nursing support/care coordination to optimize consumer outcomes and improve consumer’s quality of life through clinical support services.

EDUCATION:

Associate's or bachelor's degree from an accredited university

EXPERIENCE/KNOWLEDGE:

One to two years’ experience as a Registered Nurse, preferred experience working with individuals with mental illness. Must possess excellent computer and interpersonal skills, punctuality, ability to organize and manage several projects/tasks simultaneously and work independently.

LICENSURE/CERTIFICATIONS:

Licensed by the Michigan Department of Commerce or Professional Regulation as a Registered Nurse.
