Position Summary Provide high-quality care for long-term care residents by assessing, diagnosing, treating, and coordinating care alongside physicians and the interdisciplinary team.

Key Responsibilities

• Conduct adult/geriatric assessments and physical exams
• Manage acute and chronic health conditions
• Collaborate with physicians, families, and care teams
• Support care transitions and treatment planning
• Participate in quality improvement initiatives

Qualifications

• Active NP or PA license in the state of practice
• Board certification (AANP, ANCC, or equivalent)
• Minimum 2 years of clinical experience
• Active DEA license
• Current NPI and Medicare provider enrollment
• CPR certification
