Original job description

Harmony Health is a Tribally owned wellness clinic that treats a broad-spectrum population of Native and Non-Native patients. Harmony is established to provide those with substance use disorders whole person care: primary care, infectious disease, and mental health care. We are building a team of dedicated and compassionate individuals who align with this mission.

Harmony Health brings an innovative model of highly individualized, compassionate care to patients with -substance use disorder. From the moment someone walks through the door of our state-of-the-art outpatient facility, they will encounter our thoughtfulness and intention for a humanizing and respectful experience with us. The comforting and safe setting of our nature-inspired facility and the friendly, professional attitude of our staff help to define our journey with each patient - and with each other - from day one.

Each employee at Harmony Health creates their own ripple effect in the larger circles of the families and communities that we touch.

We are currently seeking a Nursing Manager to support our clinic in Perris, CA. This position will report to the Clinic Medical Director and will work in collaboration with clinic leadership and OTS leadership.

Indian Preference applies. Qualified Indian (American Indian or Alaskan Native) applicants will receive preference in accordance with 25 U.S.C. § 5323 and 25 U.S.C. § 472a.

POSITION SUMMARY

The Nurse Manager owns the nursing service line for the clinic. This role is accountable for nursing staffing and performance, nursing policy and practice, nursing practice in the dispensary, controlled substance compliance, competency programs, and nursing quality outcomes. The Nurse Manager provides clinical nursing oversight of RNs and LVNs, administrative and competency oversight of Medical Assistants, and administrative supervision of front office (Patient Services Representative) staff, and provides direct patient care only as needed to maintain safe operations. Supervision of front office staff is administrative and does not extend clinical duties to that role.

The Nurse Manager reports to the Director of Nursing. Until a Director of Nursing is in place, or when that position is vacant, authority is divided as follows and documented in the signed interim delegation in the Nursing Responsibility Matrix: administrative reporting (day-to-day direction, operations, scheduling approvals, and time off) is to the Clinic Medical Director; clinical medical oversight remains with the Clinic Medical Director; nursing professional-practice authority (nursing standards, competency, and scope-of-practice decisions) rests with the OTS Vice President of Nursing; and the Nurse Manager's performance evaluation and any corrective action are completed by the OTS Vice President of Nursing with Medical Director input and People Operations review. The OTS Vice President of Nursing sets system nursing standards and is the escalation authority for nursing practice.

THE DYAD MODEL

The Clinic is led through a dyad leadership model consisting of the Clinic Director and the Medical Director. The Nursing Manager works closely within this leadership framework to align nursing workflows, clinical operations, and integrated care delivery with overall clinical governance and medical standards.

KEY RESPONSIBILITIES

Responsibilities include, but are not limited to, the following:

Nursing Leadership & Workforce Management

  • Direct hiring, performance management, and corrective action for nursing, Medical Assistant, and front office staff within the approval limits of the Nursing Responsibility Matrix and Tribal employment policies; front office supervision is administrative only.
  • Recommend final written warnings, terminations, RN Supervisor hires, and new positions to the designated approver.
  • Build and manage nursing, MA, and front office schedules, staffing levels, and coverage.
  • Implement the nursing orientation, competency validation, and annual skills verification program
  • Provide administrative supervision of Medical Assistants while ensuring MA technical services remain under required licensed-practitioner supervision.
  • Apply Just Culture principles and develop future nursing leaders.

Dispensary & Controlled Substance Oversight

  • Oversee nursing practice in the dispensary as permitted by California law, including medication administration and assigned dispensary coverage; California nurses prepare, release, or transfer doses for patients in custody only after documented site-specific compliance approval, because administration authority is not treated as authority for every dispensing or distribution activity. The Medical Director retains OTP regulatory accountability.
  • Oversee controlled-substance inventory, counts, reconciliation, and diversion prevention; immediately escalate suspected theft or significant loss to the DEA registrant and designated compliance leadership, support investigations, and prepare required reports for registrant review and submission within applicable regulatory deadlines, including initial DEA notification within one business day and DEA Form 106 within 45 days (21 CFR § 1301.76(b)). The DEA registrant, the individual authorized to sign and submit DEA reports, and the escalation sequence are named in the Nursing Responsibility Matrix and the clinic's controlled-substance procedure; the Nurse Manager signs or submits DEA reports only if formally designated in writing by the registrant.
  • Maintain clear separation between nursing duties and prescribing and OTP Medical Director authority, including the written delegation framework for take-home determinations.
  • Maintain survey readiness for DEA, state, SAMHSA, and accreditation inspections.

Clinical Practice, Policy & Quality

  • Develop and maintain site nursing procedures in collaboration with the Medical Director and in alignment with OTS nursing standards.
  • California: maintain standardized procedures with the Medical Director where required for RN functions that overlap medical practice
  • Own the nursing workflow for critical laboratory result escalation and closed loop follow-up
  • Ensure incidents are reported, reviewed, and closed, and carry QAPI action items to completion.
  • Track nursing quality metrics and report results to the dyad.
  • Lead infection control, employee health, and emergency preparedness.

Operations & Escalation

  • Provide budget input for nursing staffing, supplies, and equipment.
  • Escalate urgent patient-safety issues immediately to the on-site provider or Medical Director, then notify nursing leadership.
  • Escalate nursing practice, licensure, and standards concerns through the Director of Nursing (where in place) and the OTS Vice President of Nursing
  • Provide direct patient care and dosing coverage as needed; the primary duty of this role is management of the nursing service line.
  • Fulfill mandatory reporting obligations under California law and the Indian Child Protection and Family Violence Prevention Act, and follow clinic procedures for reporting suspected abuse or neglect.
  • Other duties as assigned consistent with the role and clinic policy.

QUALIFICATIONS

Required Qualifications

  • Active, unrestricted California RN license (Board of Registered Nursing)
  • Bachelor of Science in Nursing (BSN)
  • Minimum 5 years of clinical nursing experience
  • Minimum 2 years of nursing leadership, including hiring, scheduling, and performance management.
  • Experience with controlled substance handling, inventory, and recordkeeping
  • Active American Heart Association (AHA) Basic Life Support (BLS) certification
  • Ability to successfully complete employment-related background screening, occupational health requirements, and any legally permissible drug screening, consistent with applicable federal, state, and tribal laws and clinic policy
  • Ability to provide health clearance and tuberculosis (TB) clearance.
  • Valid driver's license

Preferred Qualifications

  • Master's degree in Nursing (MSN), Healthcare Administration (MHA), Business (MBA), or Public Health (MPH)
  • 2+ years of experience in an OTP or substance use disorder setting
  • Experience leading a team through DEA, state, or accreditation surveys.
  • Just Culture certification or formal nursing leadership training
  • Experience working with tribal health programs or multicultural communities.
  • Bilingual fluency in English and Spanish or a language native to the community served

SKILLS

The ideal candidate embodies the following:

  • Strong leadership, team building, and accountability skills
  • Expertise in controlled substance compliance and survey readiness
  • Data-driven approach to quality, staffing, and process improvement
  • Clear, collaborative communication with providers, operations leaders, and staff
  • Sound judgment under pressure.
  • Commitment to patient dignity, privacy, and safety
