Job Description

Title: Case Worker

Classification: FT/Non-Exempt N4

Reports to: Program Director

Job Summary: The Case Worker is responsible for providing on-going support, advocacy and guidance for students to assist them in dealing with the personal, legal and social service needs that prevent them from being successful in developing a career path; with the students assistance and cooperation, the case manager will develop an individual plan for each student.

Job Function:

• Assist in developing the Individual Development Plan for each student.
• Provide individual and group support for the students.
• Advocate with human service, health, court and criminal justice systems to assist the students in resolving their issues.
• Meet regularly with other staff to develop and maintain a consistent approach to supporting and expanding the personal growth and leadership abilities of the students.
• Maintain detailed records on all contacts with the students.
• Teach life skills and support classes as assigned; participate in special projects as required.

Qualifications:

• Related experience working with young adults.
• Ability to establish rapport and relate sensitively to the needs of the students; understanding the needs of the community where the students reside.
• Ability to build and maintain lasting relations with a wide variety of human service agencies.
• Strong commitment to helping young people succeed in an innovative youth program, to assist them in reorienting their lives, nurturing their leadership skills and enabling them to make a difference in their community; ability to relate sensitively to a diverse group of young people.
• Excellent verbal and written communication skills; excellent interpersonal skills with a non-judgmental approach and style; ability to work as part of a team.
