Healthcare Advocate / Care Coordinator Clergy Care | Diocese of Rockford, IllinoisFULL-TIME • $55,000–$60,000 ANNUALLY • ROCKFORD, ILLINOIS

Help care for those who have spent their lives caring for others. Meta Care is looking for a compassionate, dependable healthcare professional who enjoys helping people, solving problems and building lasting relationships. You will serve as a trusted local resource for priests throughout the Diocese of Rockford. This is a full-time, relationship-based role - not a traditional bedside nursing position. You will connect personally with priests, help them navigate healthcare and benefits, and make sure important needs do not fall through the cracks. What makes this role special You will have the time to listen, the independence to help people directly, and the support of Meta Care’s broader team of nurses, pharmacists, nutritionists, and other specialists. Our goal is simple: help every priest receive the right care, in the right place, at the right time. What you will do

  • Build relationships through regular phone outreach and personal visits at parishes, homes, hospitals and other care settings.
  • Help priests locate providers, schedule appointments, understand health benefits and resolve everyday healthcare obstacles.
  • Coordinate practical support during hospitalization, discharge, rehabilitation and transitions in care.
  • Identify concerns early and connect members with appropriate clinical, community and Meta Care resources.
  • Encourage preventive care and available wellness programs; follow through until members have the support they need.
  • Work collaboratively with Meta Care colleagues and authorized diocesan contacts while respecting member privacy.
  • Document outreach, referrals and follow-up accurately in Meta Care’s care-management system. What you will bring
  • At least two years of experience in healthcare, patient support, medical offices, care coordination, case support or a related setting; comparable hands-on experience will be considered.
  • A warm, approachable communication style, sound judgment and a high level of discretion.
  • Strong organization, follow-through and comfort managing several member needs at once.
  • Ability to work independently in the community and use Microsoft Office and electronic documentation tools.
  • RN/LPN experience is welcome but not required; this is a non-clinical care coordination and advocacy role.
  • Ability to travel throughout the assigned diocesan area and conduct in-person visits.
  • Commitment to serving in a manner consistent with Catholic Social Teaching and the Ethical and Religious Directives for Catholic Health Care Services.

Successful completion of required background checks and screenings is required.Compensation and benefits

  • Annual salary: $55,000–$60,000
  • Medical, dental and vision insurance; Meta Care pays 90% of individual medical insurance premiums
  • Retirement plan; seven paid holidays; vacation and sick leaveAbout Meta Care Clergy Care is a Meta Care, Inc. program supporting the health and well-being of priests through trusted local relationships, confidential advocacy and healthcare navigation.
