Job Summary:

Lake Pointe Rehabilitation & Nursing Center, located in beautiful Conneaut, Ohio, is seeking a dedicated and compassionate Registered Nurse to join our team of rehabilitation professionals. As a Registered Nurse at Lake Pointe, you will play a vital role in delivering high-quality patient care to our short-term rehabilitation residents. Responsibilities:

• Provide comprehensive nursing care to our short-term rehabilitation residents in collaboration with our interdisciplinary team
• Develop and implement individualized care plans tailored to each resident's unique needs and goals
• Conduct patient assessments, develop and implement care plans, and evaluate patient progress
• Collaborate with rehabilitation therapists, physicians, and other healthcare professionals to ensure seamless care coordination
• Educate patients and their families on care plans, medications, and treatment options
• Participate in quality improvement initiatives to ensure optimal patient outcomes
• Maintain accurate and thorough patient records

** qualifications:**

• Current Registered Nurse license in the state of Ohio
• Bachelor's degree in Nursing (or related field)
• Minimum of 1 year of experience in rehabilitation nursing (or related field)
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
• Compassion and empathy for patients and their families
