A facility located near Sparta, Wisconsin is seeking a full-time, permanent Physical Therapist to join their team! Details are below.

Pay: $38.00-$58.00/hourly

Physical Therapist Job Opportunity:

• Full-time, direct hire opportunity
• Mon-Fri, 8-hour shifts
• Setting: Outpatient Rehabilitation
• Great therapy leadership and administration team

• Physical Therapist performs comprehensive physical therapy evaluations and establishes patient-centered treatment goals
• Monitors patient response to treatment, identify contraindications, and modify plans of care to ensure safety and effectiveness
• Collaborates with other team members and families to produce high quality of care

Physical Therapist Minimum

Qualifications:

• Current Wisconsin Physical Therapist license
• Current BLS certification
• Pelvic floor training or interest in training is highly desirable
