Job Description

The Clinical Care Manager is responsible for coordinating care and advocating for members with complex medical, behavioral health, and social needs. This role conducts comprehensive assessments, develops individualized care plans, and connects members to appropriate healthcare and community resources. The Care Manager collaborates with providers, interdisciplinary teams, and community partners to improve health outcomes and reduce barriers to care. Using clinical judgment, crisis intervention skills, and evidence-based guidelines, they support members through complex health challenges and promote engagement in their treatment plans. The position also focuses on quality improvement, regulatory compliance, and cost-effective care management. Success in this role requires strong clinical expertise, communication skills, and the ability to work independently while managing multiple cases.
