PTA

Homeside Rehab is seeking compassionate and motivated Physical Therapist (PT) and Physical Therapist Assistants (PTAs) to provide one-on-one home-based therapy services throughout Sullivan County, NY, including Swan Lake, Fallsburg, Woodridge, and Monticello.

Job Responsibilities

• Provide skilled PT treatment services under the direction and supervision of a licensed Physical Therapist or Occupational Therapist
• Carry out established plans of care and individualized treatment programs
• Provide therapeutic exercises, functional activities, mobility training, and other interventions within your professional scope of practice
• Monitor patient progress and communicate changes or concerns to the supervising therapist
• Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
• Communicate professionally with patients, caregivers, supervising therapists, and office staff
• Provide one-on-one therapy services in patients' homes throughout assigned service areas

Job Qualifications

• Active New York State PT OR PTA license and regestration
• Basic computer proficiency for electronic documentation
• Strong interpersonal and communication skills
• Ability to work independently while following the established plan of care and clinical supervision requirements
• Valid driver's license and reliable transportation for home visits
• Home health, outpatient, rehabilitation, or skilled nursing experience preferred
• Minimum of 1 year of clinical experience preferred

Job Type: Full-Time, Part-Time, Per Diem Location: Sullivan County, NY – Swan Lake, Fallsburg, Woodridge, Monticello, and surrounding areas Pay: $45–$70 per hour, based on discipline, experience, and case assignment Schedule: Flexible – choose your availability Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
