Clinical Pharmacist - Medina, OH

HealthCare Support is seeking a Clinical Pharmacist to support a well-known Closed-Door Hospice Pharmacy Provider in Medina, OH!

Step into a rewarding role where your clinical expertise truly makes a difference. You’ll serve as a trusted medication expert, collaborating closely with nurses, physicians, and hospice care teams to deliver thoughtful, patient-centered recommendations.

Compensation:

• $65- $68/hr. (depending on experience)

Shift:

• Monday–Friday: 10:00 AM – 6:00 PM with occasional weekend coverage (12–4 PM)

Daily Responsibilities:

• In this role, you will provide clinical consultative services to patients while offering therapeutic recommendations to nurses, physicians, and other hospice team members. Responsibilities include maintaining up-to-date patient profiles, including medical history, and current medications (prescription and OTC), supervising the preparation of IV medications, and automated machines to prepack medications.

Qualifications:

• Bachelor’s Degree in Pharmacy
• Clear and Active Pharmacist license in OH
