Care Delivery LPN

HealthCare Support is actively seeking a Care Delivery LPN to fill an opening in springfield, Massachusetts.

Daily Responsibilities for Care Delivery LPN

• Assists patients before and after examination, including dressing and undressing, and positioning and transfers, which may include the use of a lift.
• Measures and records patients' vital signs as required, obtains and documents patient history, and documents standardized screening assessments such as a PHQ9.
• Observes patients, charts and reports changes in patients' conditions, such as adverse reactions to medication or treatment and takes any necessary action.
• Monday to Friday 8AM - 5PM

Required Qualifications for Care Delivery LPN

• Current unrestricted clinical license as an LPN in Massachusetts required
• Current CPR Certification required
• High School or GED completion required

Pay Details: $30 - $34/ Hour
