Job Summary:

We are seeking a compassionate and experienced Licensed Hospice Social Worker to join our Hospice Agency. As a valued member of our healthcare team, you will provide emotional, social, and spiritual support to patients and their families, promoting comfort, dignity, and quality of life.

About the Role:

As a Licensed Hospice Social Worker, you will work closely with patients, families, and healthcare teams to address the physical, emotional, and spiritual needs of individuals facing end-of-life care. Your responsibilities will include:

• Assisting patients and families with health-related financial, social, and emotional concerns, and teaching effective coping skills.
• Providing social work services in accordance with the plan of care.
• Communicating changes in patient clinical status to other healthcare team members and care center office staff in a timely manner.
• Educating patients, family members, and team members on medical social work interventions and treatment plans.
• Performing care planning, transfer, and discharge functions.
• Identifying risks or threats to patients' health in their environment and implementing appropriate interventions to reduce risks.

Qualifications:

To be considered for this role, you must:

• Hold a master's or doctorate degree from a school of social work, accredited by the Council on Social Work Education.
• Have at least one year of social work experience in a healthcare setting.
• Meet state and federal requirements for this position.
• Hold current CPR certification.
