Original job description
Provide support and skill development based on the Plan of Services and Support. Assist with activities of daily living, communication, socialization, and community resources. Complete daily documentation including service notes, date collection, and incident report. Ensure health and safety through personal care, monitoring changes emergenct response and incident reporting. Must have 2 years of experience. Experience working with individuals with intellectual disabilities. High School Diploma or GED required.
