What you'll do

• Complete psychosocial assessments and participate in care planning
• Support residents and families with adjustment, concerns, and emotional needs
• Coordinate discharge planning and connect residents with community resources
• Advocate for residents' rights, dignity, and quality of life
• Participate in interdisciplinary team meetings
• Maintain accurate, timely documentation in line with state and federal regulations
