RN Hospice Admissions

RN Hospice Admissions (Excellent Full Benefits Package, Competitive Compensation Package and more!) We are currently hiring for a skilled Hospice Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Position Responsibilities:

· Coordinate and oversee all aspects of new patient admissions · Collaborate with patient care teams to support seamless communication and continuity of care · Educate patients and families regarding condition, needs, and care · Perform head-to-toe physical assessments · Complete all required documentation accurately and on time

· Current Registered Nurse license · Current driver's license · Current CPR certification · Two years of Registered Nurse experience in a clinical care setting · One year of full-time hospice experience · Reliable transportation and valid automobile liability insurance
