ESO Consultants Is looking for Registered Nurses to fill the role of RN - SNF, to work in hospitals in New York.

Shift:

• Hours Per Week - 40
• Hours Per Day - 08
• Days Per Week - 05

Vacancy:

• New York - 20

Remuneration:

• $2200 - $2700 Per Week

Qualifications:

• 1 year of LTC and Med/Surg experience is required
• RN License - required
• Primary Source Verification - required
• AHA BLS - required
• IV certification - preferred

Responsibilities:

• Maintains an environment of safety for patients, self and others.
• Assesses patient's condition, including biophysical, psychosocial and age specific needs.
• Documents patient's status according to unit specific procedure.
• Develops an individualized plan of care utilizing the nursing and medical diagnosis. Collaborates with other health care team members in formulating a plan.
• Implements the nursing plan of care on assigned patients.
• Evaluates patient's status, nursing instructions, appropriateness of the nursing plan of care and outcomes of the care provided.
• Initiates a patient education plan according to the individualized needs of the patient and as prescribed by the physician and/or policy; includes patient and family instructions, as well as, discharge planning.
• Maintains educational requirements as required by the Board of Nursing.
