Regional MDS Coordinator (RN): Central New York Supporting Elderwood’s Brockport, Liverpool, Hornell and Waverly communities (regional travel required)

Are you an MDS expert who likes being the person teams turn to? Elderwood is looking for an experienced Regional MDS Coordinator to support four of our skilled nursing communities across Central and Western New York.

In this role, you’ll be the go-to resource for MDS accuracy, reimbursement and quality at Brockport, Liverpool, Hornell and Waverly. You’ll divide your time among the four sites, working side by side with nursing and facility teams to audit, teach and solve problems. You’ll report to our Director of MDS and Clinical Reimbursement and be part of a corporate team that invests in your growth and stays current on every CMS change, so you’re never figuring it out alone.

If you enjoy variety and coaching others, and want your expertise to make a visible difference in resident care and facility outcomes, we’d love to talk with you.
