What You’ll Do

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns.
• Teach patients and caregivers self-care techniques.
• Partner with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and maintain medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to coordinate care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive nursing care.

What You’ll Need

• Current state license as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• At least 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Hospice or home health experience preferred.
