Original job description

Healthcare works best when every person on the team is pulling in the same direction, and that’s the heart of this role. As a Registered Nurse at our Health Center, you’ll be the steady, caring presence that helps patients feel seen, heard, and well cared for. You’ll also keep the Health Center humming in sync with the rest of East Bay Community Action integrated health and family programs.

You’ll help solve the everyday barriers that keep people from getting the care they need: confusing transitions after a hospital stay, gaps in preventive care, delays in prior authorizations, and missing community supports. Your work moves our mission forward one patient, one handoff, and one follow-up call at a time.

What You'll Do

Deliver great care

  • Conduct comprehensive health assessments that uncover patient needs, barriers, and behavioral health concerns
  • Provide skilled nursing care: immunizations, wound care, chronic disease management, and patient education that helps people stick with their treatment plans
  • Handle phone and walk-in triage so patients get to the right care and avoid unnecessary ER visits
  • Administer and reconcile medications per provider orders, and watch for effectiveness and side effects

Connect the dots

  • Coordinate care across departments and outside providers, including medication reconciliation within 48 hours of a hospital discharge or specialist visit
  • Use population health dashboards and registries to reach out to patients who are due for chronic disease management, preventive services, or care gaps
  • Manage prior authorizations, records requests, and lab follow-up so patients aren’t left waiting
  • Link patients to behavioral health, pharmacy assistance, social services, and community resources

Keep it all running smoothly

  • Document in NextGen EHR to meet HRSA, RIDOH, and insurance requirements
  • Review transition-of-care documents within 24-48 hours and keep patient registries (anticoagulation, diabetes, hypertension, and more) current
  • Support your assigned primary care provider(s) across chronic disease management, behavioral health integration, and maternity services
  • Follow infection control practices and speak up about quality or safety concerns
  • Help shape EHR workflows, clinical decision support tools, and e-prescribing protocols

Be a great teammate

  • Join daily huddles, team meetings, case reviews, and peer quality reviews
  • Provide cross-coverage for fellow nurses across pods, teams, and sites
  • Build relationships with medical providers across our integrated service network
  • Protect patient confidentiality, dignity, and rights in line with HIPAA

What You'll Bring

Required

  • Associate’s Degree in Nursing (or equivalent)
  • 2+ years of experience in community health, acute care, or emergency/urgent care
  • Experience working with primary care providers on care coordination and disease management
  • Current RI Registered Nurse (RN) license
  • Current BLS/CPR certification
  • Working knowledge of HIPAA, electronic health records, medication administration protocols, care coordination, and disease management protocols

Bonus points

  • Bachelor’s Degree in Nursing (BSN)
  • Experience in a Federally Qualified Health Center (FQHC)
  • Bilingual in Spanish, Portuguese, or Cape Verdean Creole

Who thrives here: People who take ownership, sweat the details, adapt with grace, collaborate across disciplines, listen actively and communicate in a culturally responsive way, and find practical solutions to barriers to care. Above all, the people who thrive bring joy to their work because this is the work they want to do and the place they want to be.
