Posting Date 11/04/202612800 Shaker BlvdSuite 1, Cleveland, Ohio, 44120, United States of AmericaDSD Modality Work Address: 9014 Cedar Ave, Cleveland, OH 44106 Set 3-day work weeks (~12-hour shifts) with NO overnight on-call and NO Sunday treatments.

Ready to Transition into Nephrology & Specialized SNF Dialysis? Are you an experienced Registered Nurse looking for direct patient connection, true clinical autonomy, and a predictable schedule without the chaos of rotating hospital shifts, forced overtime, or overnight call? DaVita SNF Dialysis (DSD) is launching a brand-new on-site dialysis den conversion suite at Foundations Gardens of Fairfax near Cleveland’s University Circle. Due to the high level of clinical autonomy required in our post-acute den setting, candidates must have at least 1 full year of general nursing experience, but prior dialysis experience is NOT required. Through our renowned New to Dialysis program, we provide fully paid, comprehensive clinical training to teach you everything you need to become a confident, autonomous dialysis leader.

About DaVita SNF Dialysis (DSD) Approximately 10% of all dialysis patients reside in a skilled nursing facility (SNF) and are transported to outpatient clinics three times a week - placing a significant physical burden on patients. DaVita SNF Dialysis brings high-quality renal replacement therapies directly on-site inside an intimate, modern 6-chair dialysis den. In this role, you partner directly with a dedicated Patient Care Technician (PCT) to deliver life-sustaining hemodialysis care to residents right inside their facility - backed by the full clinical governance, technology, and support of DaVita.

What to Expect as an NTD (Paid Comprehensive Training) You do not need prior nephrology or dialysis experience to succeed. DaVita’s industry-leading training curriculum is designed specifically to support experienced nurses transitioning from other clinical specialties into dialysis:

  • Dedicated 9–12 Week Paid Onboarding: A balanced combination of structured classroom theory, virtual modules, and 1-on-1 precepted hands-on training with tenured dialysis nurse educators.
  • Mastering Nephrology & Technology: Learn sterile cannulation of complex AV fistulas/grafts, central venous catheter (CVC) care, hemodialysis physiology, and the operation and troubleshooting of Fresenius 2008T dialysis machines and Reverse Osmosis (RO) water treatment systems (both central water rooms and portable RO units).
  • Gradual Ramp to Autonomy: Build clinical confidence under the continuous guidance of a dedicated preceptor before independently leading your own den suite.

What You’ll Do Once Trained

  • Autonomous Clinical Leadership: Lead treatment operations within an intimate 6-chair den setting. Perform comprehensive pre- and post-dialysis patient assessments, evaluate fluid removal orders, monitor hemodynamic stability, and manage vascular access.
  • Direct Team Collaboration: Direct and partner closely with your dedicated Patient Care Technician (PCT), fostering strict infection control, sterile protocols, and continuous patient safety.
  • Technical & Water System Oversight: Oversee dialysis machine setup and ensure daily water quality testing and infection control across central water rooms and portable RO systems.
  • Holistic Care Coordination: Collaborate directly with SNF facility medical staff, attending nephrologists, dietitians, and your Regional Operations Manager (ROM).

Why Choose DSD at Gardens of Fairfax?

  • Predictable Schedule & Work-Life Balance: Set 3-day work weeks (~12-hour shifts) with NO overnight on-call and NO Sunday treatments.
  • Meaningful Resident Continuity: Care for the same resident patients week after week, building deep therapeutic rapport and observing the direct impact of your care.
  • Boutique Suite Autonomy: Enjoy the calm, focused setting of a small 6-chair den while having the full backing, safety resources, and clinical leadership of DaVita.
