Original job description
Position Summary:
The RN Nurse Coordinator Lead is a working senior RN in the ICF program who provides day-to-day clinical guidance and work direction to the RN Nurse Coordinators, in accordance with the policies and practices of the agency and in adherence with all applicable laws and/or regulations. The Lead supports consistent nursing practice and regulatory readiness across the ICF programs and fosters collaborative relationships with Administration and Clinical Operations.
The Lead will contribute input to performance evaluations, personnel actions, and staffing decisions as requested by the Director of Nursing. The Director of Nursing retains authority over performance evaluations, staffing, personnel actions, nursing policy and program-level accountability.
Principal Duties and Responsibilities:
- Maintain a defined nursing caseload assigned by the Director of Nursing, providing nursing care, assessments (including for new admissions) and individualized health care plans per the Nurse Practice Act and ICF/IID regulations.
- Monitor the health status of individuals on the caseload, respond to emergent needs, and participate in the nursing on-call rotation.
- Coordinate medical interventions and follow-up with the Medical Director, Licensed Nurse Practitioner and other providers, and contribute nursing input to interdisciplinary team meetings
- Serve as a nursing contact for external partners such as hospitals, specialists, pharmacies and guardians, sharing health information accurately and in keeping with HIPAA and confidentiality requirements.
- Serve as first point of clinical escalation for the RN Nurse Coordinators, providing day-to-day guidance and escalating urgent or complex matters to the Director of Nursing or Medical Director per protocol.
- Meet routinely with the RN Nurse Coordinators to review open health concerns and workload; report barriers and proposed solutions to the Director of Nursing and provide backup coverage as necessary.
- Mentor and precept new RN Nurse Coordinators and validate competencies using approved tools; give the Director of Nursing performance input when requested.
- Foster collaborative, respectful relationships with stakeholders in Administration and Clinical Operations: listen to their concerns, share information proactively, seek input before changes that affect their programs, and resolve disagreements constructively. The strength of these relationships is a measure of success in this role.
- Complete sample-based electronic health record reviews (medication administration, treatment records, health care plans) on the Director of Nursing's audit schedule; review incidents and variances and support survey readiness.
- Coordinate or deliver staff health training as scheduled by the Director of Nursing, using approved materials.
- May perform other duties as assigned.
Requirements

- Sound independent clinical judgment within RN scope; strong communication and mentoring skills, including guiding peers without formal authority; and demonstrated ability to build collaborative, trusting relationships with Administration and Clinical Operations and to incorporate differing perspectives.
- Proficiency in English and electronic health records; discretion in all interactions; commitment to person-centered, trauma-informed, recovery-oriented and culturally responsive care.
Physical Demands:
Ability to perform physical interventions with individuals, including lifting, repositioning and assisting with self-care; ability to stand, walk and bend for extended periods.
