Client Services Assistant

Examples of Duties

• Perform all resident care as assigned and according to individualized service plan, facility policies and procedures and participate in care conferences of residents.
• Assist residents per individual support plan direction with;
• Tub bath or shower.
• Oral, hair and nail care, shaving, dressing and undressing, application of make-up.
• Elimination needs including toileting or offering verbal prompting to remind resident to use the bathroom.
• Answer all resident requests for assistance promptly.
• Give emotional and social support, and attend to the resident's physical comfort as appropriate.
• Administer resident oral and topical medications as ordered and per training.
• Takes and record temperature, pulse, respiration, and blood pressures.
• Assist residence with meal times, according to established protocols, maintain intake and output as requested, weigh residents as instructed.
• Observe and report physical and/or behavioral changes in resident immediately to the Program Specialist.
• Document appropriately regarding observations.
