Original job description

Advanced Practice Same Day Access Practitioner Job Summary: The Advanced Practice Clinician will provide comprehensive medical care to patients served at Choptank Community Health System (CCHS), in accordance with the policies and procedures and philosophy of CCHS. All duties and responsibilities are performed across life cycle (infants, children, adolescents, adults, and geriatric), as applicable. This is an exempt, full-time position in pay grade 9 with the pay range of $94,056.00 - $141,083.00. The Advanced Practice Clinician reports directly to the Chief Clinical Officer. This position supports CCHS’s same-day access model, with a primary focus on acute illness and other time-sensitive care needs. This may include high urgency hospital or ER follow up visits and high-priority quality measures such as uncontrolled hypertension or diabetes – this is when the PCP is unable to address the patient need in the necessary timeframe. Based on availability and need, the clinician may also assist with performing Annual Medicare Wellness visits to complement the care of the PCP. The clinician serves as a bridge, addressing the clinical need, collaborating with the PCP when needed, and ensuring the patient gets connected back to the PCP. The clinician uses available capacity to improve timely access while advancing quality and continuity with the patient’s primary care team. Clinical and Operational Accountability The Chief Clinical Officer remains the direct supervisor, with responsibility for clinical oversight and performance. Operations Directors and Practice Managers oversee paid leave approval, timecard review and approval, work schedules, appointment templates, and daily operations in accordance with CCHS policies. The clinician works with both clinical and operational leaders to coordinate coverage and patient access. Concerns affecting both clinical care and operations are addressed jointly by the appropriate leaders.
