Licensed Practical Nurse (LPN) Berkeley Health and Rehab – Berkeley, IL Berkeley Health and Rehab is seeking a compassionate, dependable, and dedicated Licensed Practical Nurse (LPN) to join our nursing team! Responsibilities
- Provide direct nursing care to residents in accordance with physician orders, care plans, and facility policies.
- Administer medications and treatments as prescribed.
- Monitor residents' health conditions and report changes to the appropriate nursing personnel.
- Perform nursing assessments and document care accurately and timely.
- Assist with developing, implementing, and updating resident care plans.
- Communicate with residents, families, physicians, and other healthcare professionals.
- Supervise and support Certified Nursing Assistants (CNAs) in providing quality resident care.
- Maintain compliance with infection control standards and applicable regulations.
- Promote residents' dignity, comfort, safety, and overall well-being.
Qualifications
- Current Illinois Licensed Practical Nurse (LPN) license required.
- Previous experience in a skilled nursing or long-term care setting preferred.
- Strong clinical, organizational, and communication skills.
- Ability to work effectively as part of a healthcare team.
- Compassionate approach to resident care and a commitment to quality service.
Schedule
- AM Shift: 7:00 AM – 3:00 PM
- PM Shift: 3:00 PM – 11:00 PM
- NOC Shift: 11:00 PM – 7:00 AM
- Rotating weekends required
