Sales Specialist

Ayerdi is seeking a compassionate and detail-oriented Case Manager to support clients by assessing needs, coordinating services, and advocating for resources. This role requires strong problem-solving and organizational skills to ensure individuals receive comprehensive support tailored to their circumstances.

Key Responsibilities:

• Conduct client assessments to determine needs and develop individualized care plans.
• Coordinate with service providers, healthcare professionals, and community organizations.
• Advocate for clients to access essential services, including housing, healthcare, and employment support.
• Maintain detailed case records in compliance with company and regulatory standards.
• Monitor client progress through regular follow-ups and make necessary adjustments to care plans.
• Collaborate with internal teams to improve service delivery and client outcomes.

Qualifications:

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field.
• years of experience in case management, social services, or a related field.
• Excellent communication, problem-solving, and crisis intervention skills.
• Familiarity with community resources and case management software.
• Ability to work independently and within a team to support client needs.
