Responsibilities:

• Safely transport patients to and from appointments, activities, and within the day habilitation center.
• Assist patients with personal hygiene needs, restroom assistance, and mobility as required.
• Monitor and report any health concerns to the appropriate supervisor or healthcare professionals.
• Follow specialized diets, assist with meal preparation, and ensure meal supervision.
• Maintain cleanliness by performing light housekeeping duties.
• Ensure the overall care, health, and safety of individuals under your supervision.
• Complete all necessary paperwork accurately and in a timely manner.
